Persistent fatigue that does not improve with rest, joint stiffness, digestive upset, headaches, skin changes, or a general sense that your body is not recovering well can all raise the same question: could inflammation be involved? The best tests for chronic inflammation are not necessarily the longest list of laboratory markers. They are the tests that help connect your symptoms, health history, lifestyle, and physical findings to a clear next step.
Inflammation is a normal part of healing. It helps the body respond to injury and infection. The concern is not inflammation itself, but an inflammatory response that remains elevated, repeats frequently, or develops alongside an underlying concern such as autoimmune activity, insulin resistance, digestive dysfunction, nutrient imbalance, chronic infection, or environmental exposure.
A thoughtful evaluation starts by understanding the person, not just the lab report. Test results are most useful when interpreted in the context of how you feel, what you eat, how you sleep, your stress load, medications, medical history, and family history.
Why Chronic Inflammation Can Be Difficult to Identify
Chronic inflammation does not always produce obvious signs. Some people have a clearly elevated inflammatory marker. Others have symptoms that are consistent with inflammation while routine blood work appears normal. Neither situation should be interpreted in isolation.
Markers can rise temporarily after a viral illness, a strenuous workout, surgery, poor sleep, dental infection, or even a flare of a known condition. Conversely, a normal result does not rule out a localized issue in the gut, joints, gums, or another tissue. This is why testing should be purposeful rather than a search for a single number that explains everything.
Best Tests for Chronic Inflammation: The Starting Point
For many adults, a foundational blood work review provides the most appropriate place to begin. These tests can identify broad inflammatory patterns, screen for related health concerns, and determine whether more targeted evaluation is warranted.
C-Reactive Protein and High-Sensitivity CRP
C-reactive protein, or CRP, is made by the liver in response to inflammation. A standard CRP test is commonly used when more active inflammation or infection is suspected. High-sensitivity CRP, often called hs-CRP, measures lower levels of CRP and may be useful when assessing low-grade inflammation in the context of cardiovascular and metabolic health.
An elevated CRP does not identify the cause. It is a signal to look further. Recent illness, injury, excess body fat, smoking, uncontrolled blood sugar, inflammatory conditions, and other factors can affect the result. Repeating the test after an acute illness has resolved may be more informative than reacting to one isolated value.
Erythrocyte Sedimentation Rate
The erythrocyte sedimentation rate, or ESR, is another general marker of inflammation. It measures how quickly red blood cells settle in a tube of blood over a set period. A faster rate can be associated with inflammation, although anemia, age, pregnancy, and other factors can also influence it.
ESR and CRP are often considered together. In some conditions, one may be elevated while the other is not. Their value lies in the pattern, especially when symptoms such as prolonged joint pain, fever, unexplained weight changes, or marked fatigue are present.
Complete Blood Count and Comprehensive Metabolic Panel
A complete blood count, or CBC, looks at red blood cells, white blood cells, platelets, and related measurements. Changes in white blood cell patterns may point toward infection, immune activity, or other concerns. Low red blood cells can suggest anemia, which can contribute to fatigue and may require its own evaluation.
A comprehensive metabolic panel, or CMP, reviews markers related to liver function, kidney function, electrolytes, protein status, and blood glucose. Chronic inflammatory states can affect these systems, and liver or kidney concerns can change how symptoms and supplements should be approached. These are not inflammation tests by themselves, but they provide essential clinical context.
Ferritin and Iron Studies
Ferritin stores iron, but it can also rise as an acute-phase reactant during inflammation. A high ferritin value does not automatically mean someone has excess iron, and a "normal" ferritin level may still need closer interpretation when fatigue, hair loss, restless legs, heavy menstrual bleeding, digestive symptoms, or anemia are present.
When indicated, ferritin should be reviewed alongside serum iron, total iron-binding capacity, transferrin saturation, the CBC, and inflammatory markers. This prevents oversimplified conclusions and supports a more accurate plan.
Targeted Testing When Symptoms Point to a Specific System
The right next test depends on the clinical picture. Specialty testing can be valuable, but it should answer a meaningful question rather than add uncertainty through an overly broad panel.
Digestive Testing for Gut-Related Symptoms
The digestive tract has close communication with the immune system. For people with chronic bloating, abdominal discomfort, loose stools, constipation, reflux, food reactions, or unexplained nutrient deficiencies, a digestive evaluation may be appropriate.
A stool test may assess digestion, microbial balance, and signs that further gastrointestinal evaluation is needed. Fecal calprotectin is a more specific stool marker that can help distinguish inflammatory bowel activity from some functional digestive disorders. It is particularly useful when symptoms include ongoing diarrhea, urgency, bleeding, weight loss, or nighttime bowel movements.
Food sensitivity testing is sometimes requested by patients with inflammation concerns. It may be considered in select situations, but it should not replace a detailed food and symptom history, an evidence-informed elimination and reintroduction plan, or medical evaluation for conditions such as celiac disease. A positive food panel alone does not prove that a food is causing systemic inflammation.
Autoimmune and Joint-Focused Testing
Joint swelling, prolonged morning stiffness, rashes, mouth sores, dry eyes, unexplained fevers, numbness, or recurring symptoms that affect multiple body systems may warrant evaluation for autoimmune or rheumatologic conditions. Depending on the presentation, a clinician may consider tests such as antinuclear antibodies, rheumatoid factor, anti-CCP antibodies, thyroid antibodies, or other condition-specific markers.
These tests require careful interpretation. Some healthy individuals have positive antibody results without active autoimmune disease, while some people with autoimmune conditions initially have negative screening tests. Symptoms, examination findings, family history, and referral to the appropriate specialist remain essential.
Metabolic and Hormonal Contributors
Blood sugar dysregulation, central weight gain, poor sleep, and chronic stress can contribute to a pro-inflammatory pattern. Fasting glucose, hemoglobin A1c, fasting insulin when clinically appropriate, lipid markers, and liver enzymes can help reveal metabolic factors that may be influencing inflammation.
Thyroid testing can also be relevant when fatigue, bowel changes, temperature sensitivity, hair changes, or weight changes are part of the picture. Hormonal testing may be considered for specific symptoms, but timing and the individual’s age, cycle status, medications, and medical history matter. More testing is not always better. Well-chosen testing is better.
What to Do Before and After Testing
Preparation can affect results. Tell your clinician about recent infections, vaccinations, intense exercise, medications, supplements, alcohol use, smoking, and major sleep disruption. Do not stop prescribed medication simply to prepare for testing unless the prescribing clinician advises you to do so.
After testing, avoid treating a number instead of treating the whole person. An elevated marker may call for medical evaluation, follow-up testing, or referral.
